Hi Freddie, you do not need to put the paper towel under the lid. I do it in cases that the lid is not tight. This makes sure nothing gets in there and that the seal is nice.

One more thing Freddie, you might want to try oregano essential oil on her wound (diluted). Additionally, she could try also treating it from the inside out with an antimicrobial tinctures.
